WHAT IS THE COMMUNITY CONNECTOR PROGRAM?

The Community Connector Program is a community-based approach using trained community members to identify socially isolated older adults.

WHO ARE COMMUNITY CONNECTORS?

Ideal Community Connectors are individuals who come into contact with older adults through their work or their daily activities.

HOW CAN I LEARN TO BE A COMMUNITY CONNECTOR?

You can sign up for a workshop that educates Community Connectors about the health and social impact of social isolation and loneliness and how to refer isolated older adults to community supports.
